How can I get a Peer Support Worker?

You will need to:

l       Ask your care worker (e.g. your care

        coordinator or consultant psychiatrist) for

        a referral form.

 

2       Complete the referral form together with your care worker.

        Send the completed form to:

Please be aware that your time with your allocated Peer Support Worker will be limited, but Peer Support can be a great stepping stone onto new opportunities and linking in with the wider

Here are some examples of the more specific work we can support you with…

 

We can introduce and support you to complete a WRAP (Wellness Recovery Action Planning).

 

WRAP is a tool which can help you in your Recovery journey. It explores ways in which you can manage periods when you are unwell, and the things you can do to stay well. It is developed and owned by you, but you can also share it with your care team to help inform your care plans.

 

What is peer support?

Peer support is when people who have a lived experience of mental distress use their personal experiences to support others with similar difficulties in their Recovery Journeys.

 

Who we are

 

We are a pilot Peer Support Project in the Fareham and Gosport area.

We are funded by Hampshire County Council and we are part of Southern Health NHS Foundation Trust.

 

The project is working closely with the East area Service User Network (SUN).

 

We are also supporting the Personalisation Project in Fareham and Gosport.

 

How we can help you

 

As Peer Support Workers we are in a good position to understand what you are going through, we can offer time to listen, and an opportunity for you to talk about how you are feeling.

 

We can assist you in exploring where you are in your Recovery journey by using the Recovery Star tool

 

We can help you think about what you want to achieve and support you to set goals so that you are able to get there.

 

We can also help with Personalisation. For example if you are applying for a personal Budget, we can help you complete the Self Assessment

 

Questionnaire.

 

What do we mean by

Recovery?

 

Recovery is not necessarily about being symptom free, or never being unwell again, Recovery is an ongoing process. It can mean finding ways to manage your mental health problems, regaining a sense of control and achieving a meaningful life. Recovery is personal and individual, yet there is a thread that runs through everyone’s journey and that is Hope; Hope that Recovery is possible.
